The nature of our practice is to give our patients the utmost in care and service.   Our primary goal is to assist you in attaining and maintaining optimal oral health.  Office hours are by appointment and we do value your time. Should any unforeseen circumstance cause you to change your reserved appointment, we ask that you give us 48 hours notice to avoid any cancellation charges.  This allows us to schedule other patients in your time slot.  We will be more than happy to re-schedule an appointment for you to a more convenient time.  Appointments cancelled or rescheduled with less than 48 hours notice are subject to a cancellation fee.  Please do not use this form to make changes to your scheduled appointment.
